Cardiolotsen sind medizinisches Fachpersonal (Medizinische Fachangestellte oder Gesundheits- und Krankenpfleger) mit Zusatzqualifizierung zur individuellen Betreuung von Patientinnen und Patienten mit Herzerkrankungen. Cardiolotsen sind angestellt am Partnerkrankenhaus. Sie identifizieren geeignete Patienten während des stationären Aufenthaltes, schreiben sie in das Versorgungsprogramm ein und betreuen sie bis zu 12 Monate nach dem stationären Aufenthalt mit ihrer kardiologischen Erkrankung. Die konkreten Aufgaben des Cardiolotsen und Inhalte des Versorgungsprogramms entnehmen Sie bitte den weiteren Anlagen.
